Lower North Severe Conduct Disorder Programme: Stage 3 - Reintegration.

This service is a collaborative venture with Child Youth & Family Services, Capital & Coast District Health Board and the Ministry of Education.

Role of Youth Horizons

Youth Horizons provides stage three of the Lower North Severe Conduct Disorder Programme (LNSCDP). Our service enables the reintegration of young people returning to their families and communities from the LNSCDP unit.

The Service

The LNSCD unit, located at Epuni in Lower Hutt, provides a secure environment and clinical input for young people and their families. This is then followed by a residential home environment in the community.

After about 6 months in the LNSCDP, young people then transition to the Youth Horizons Reintegration Programme for a further 18 months. This comprises intensive clinical support for families through MST, or utilisation of Foster Carers with intensive support. This part of the service includes coordination of their placement, educational, training or vocational needs, and liaison with other agencies or community organisations involved with the young person and this service.
