Youth Horizons Trust is committed to ensuring that programme gains are sustained following discharge of the young person from Youth Horizons. Included in the Youth Horizons Trust's programme are skills that enable young people to successfully reintegrate back into the home environment or, alternatively, move on to independent living.
A relapse prevention approach is taken with the young people to give them practical skills that they can continue to use after discharge. Flexibility in length of stay allows for a slow transition from the programme. This process is facilitated and monitored by the young person’s Care Co-ordinator. For those young people who are unable to return home, they are eligible for Youth Horizons Trust's Ka Awatea service.
